Uttar Pradesh’s Personnel Department has sought an investigation after 16 Health Department employees were reportedly found with duplicate identity details on the Manav Sampada portal. The flagged records include pharmacists, dark room operators and contractual National Health Mission workers posted at different locations.
Identical Personal Details Found Across Districts
The issue emerged during a technical review of employee information stored on Manav Sampada, the state government’s digital human resource management platform.
Authorities reportedly found multiple records carrying identical names, fathers’ names and dates of birth despite the employees being posted in separate districts. The findings have raised questions about whether the discrepancies arose from technical errors, data-entry lapses or deliberate manipulation.

The Personnel Department has sent details of the suspected cases to the Health Department and requested verification. A senior-level meeting has also been convened to review the findings and decide further action.
Officials have stressed that the entries remain under examination and that no conclusion has been reached about the cause of the duplication.
Pharmacists and Contractual Workers Under Review
The flagged cases reportedly include pharmacists serving in Chandauli and Azamgarh whose records contain matching personal information.
Similar patterns were identified among pharmacists posted in Ghazipur and Auraiya, as well as employees working at health facilities in Sonbhadra and Pratapgarh.
Authorities are also examining contractual workers engaged under the National Health Mission. Some dark room operators posted in different districts reportedly have identical identity details in official records.
Further cases have been linked to employees associated with health institutions in Lucknow, Kanpur Dehat, Etawah and Agra. Each record is expected to be examined separately before responsibility is determined.
Documents and Digital Audit Trails to Be Checked
Manav Sampada stores government employee information covering appointments, postings, service books, transfers and other administrative matters. Duplicate identities could affect verification, salary payments and service records if the discrepancies are confirmed.
The review is expected to include appointment documents, service records and identity credentials. Technical teams may also examine system logs and audit trails to establish when the disputed entries were created or modified.
Cybercrime expert and former IPS officer Prof. Triveni Singh said duplicate identity records require detailed document verification, technical audits and log analysis. Such examination could help determine whether the entries resulted from system errors, administrative negligence or intentional interference.
The Personnel Department has sought factual reports from the authorities concerned. Further action will depend on the findings of the ongoing inquiry.
